PRACTICE POLICIES FOR NON-EMERGENCY SERVICES ONLY

RESERVATION POLICIES:

We only schedule non-emergency appointments and see patients by appointment in the practice. Appointment holders may reschedule once up to 5 calendar days before the appointment using the link in the appointment confirmation email. We kindly ask that you do not call the practice to reschedule appointments as this takes valuable time away from other clients. All available slots are published on the website.

CANCELLATION POLICY:

Please cancel confirmed appointments no later than 5 calendar days before your appointment by using the confirmation link provided. If the link does not work, please email the practice. If you do not cancel your appointment in time, we will bill you according to the cancellation policy. Our office reserves time slots for each client by appointment and does not routinely accept walk-in clients or wait-list requests.

CHECK-IN POLICY:

Please be sure to bring all legible and completed requested forms, medication lists, and patient records including radiology pictures and reports, medications, conditions, and diagnosis reports. Please arrive no more than 5 minutes before your scheduled appointment time. Clients who are not in the practice at the time of their appointment or online for telemedicine appointments are considered no-shows. If you no-show for your appointment we will bill you according to the check-in policy.

PARKING:

The practice and parking is available by appointment only. One parking spot is available for the practice in the courtyard directly in front of the practice door. Please drive onto the property and park only if you have a confirmed appointment within 5 minutes. Patients are kindly asked to drive out of the courtyard immediately following their appointment to accommodate the next patient. Continue driving down the driveway, turn the corner, and barrier-free parking is on your right. Be sure to pull forward ensuring other cars are able to pass your parked car and turn. Parking is forbidden on the street near the practice address. The nearest paid city parking lot is located at:

BARRIER-FREE FACILITIES:

The practice is barrier-free. The restroom and treatment rooms are located on the ground level and wheelchair accessible.

LEGAL DISPUTES:

These Terms require that should we get into a dispute with you relating to Services rendered (or vice versa), unless you live in Germany or the European Union, the dispute will be resolved by BINDING ARBITRATION. This means that YOU AGREE TO GIVE UP YOUR RIGHT TO GO TO COURT TO SUE US (OR BE SUED BY US) UNDER THIS CONTRACT (except for some disputes that can be taken to small claims court). Our disputes will be determined by a NEUTRAL ARBITRATOR and NOT A JUDGE OR JURY and you cannot start nor join a class action lawsuit. These terms specifically include United States Citizens subject to Status of Forces Agreement and/or civilian contractors and their family members.
